Throughout the school year, various forms are sent home (and subsequently lost) that need to be filled out or situations change and information needs to be updated.  Below are links to forms that are often requested.

Volunteer Driver Statement - used when driving students on a field trip.

Field Trip Permission - used when students participate in a field trip off school grounds.

Extended Day Care Agreement - required for participation in the Extended Day Care program.

Dismissal Procedure - use this form to confirm/change dismissal arrangements for your child with the office.

Acceptable Use Policy - use this form to allow your student to use academy technology.


Medical Forms

Emergency Medical Authorization - this form is sent home on the first day of school and should be updated at any point during the year when there is a change.

Medication Policy & Physician and Parent Request for the Administration of Medication by School Personnel - complete this form if your child requires medication during school hours.
